REHABILITATION DEPARTMENT:
Physical Therapists: Full Time or Part Time Weekends & Holidays
Our Rehab Department is actively involved in joint replacement, balance dysfunction, falls intervention and safe community re-entry programs. It is part of a collaborative, stimulating, and rewarding work environment that encourages continued learning and drive to excellence in patient care.
Plan, organize, and conduct physical therapy program in the patient’s home or place of residence to facilitate rehabilitation of clients. Diversified adult caseload (Orthopedics, Neuro, and Geriatrics). Requires a minimum of Bachelor’s degree in Physical Therapy and current Massachusetts PT license; one year experience required; preferably in home care. Must have current Massachusetts driver’s license and a reliable automobile. Must possess adequate strength, agility, gross and fine motor skills to accomplish responsibilities of position and required documentation.
Senior Physical Therapist – Full Time (2)
Provides clinical supervision and leadership to Physical Therapy Assistants (PTAs) for the management of patient care within a designated region. Monitors the service/outcomes and coordinates/communicates with patients, families, and physicians. Works under the general direction of the Director of Rehabilitation Services. Assignments generally entail: Direct care/service, development/implementation of patient plans of care and physician protocols within scope of practice, to include performing admissions, initial evaluations, clinical assessments, CMS mandatory reassessments, discharge assessments, and supervision of PTAs in the routine care of their patients, providing case management and follow up, to ensure care plan stated outcomes are achieved. Documents treatment(s), response, and progress according to BVNA charting protocols for the electronic medical record. Plans and prepares written care plan including goals and actions based on evaluation of patient data, including physician orders. Requirements include current Massachusetts Physical Therapist License, DPT or five years of clinical experience as a Physical Therapist in community health, a skilled nursing facility or acute care setting, with at least one year in community health required. Previous experience working with PTAs preferred; coaching and teaching skills.
Physical Therapy Assistant – Part Time
Works collaboratively with rehab team under the supervision of one or more PTs to effectively carry out individualized patient plans of care. Instructs, motivates, and assists patients in their homes or place of residence. Effectively performs functional activities and other types of rehab interventions including but not limited to transfers and gait training, balance and strengthening exercises. Assists PT in taking objective measures. Must have current Massachusetts PTA license with one year experience preferably in home care; Massachusetts driver’s license and reliable automobile. Must possess adequate strength, agility, gross and fine motor skills to accomplish responsibilities of position and required documentation.
Occupational Therapist – Full Time
Plans, organizes and conducts occupational therapy program in the patient’s home and in community programs to facilitate development and rehabilitation of mentally, physically, or emotionally disabled persons. Appropriate Degree and current Massachusetts license required; one-year experience preferred. Must have current Massachusetts driver’s license and an automobile. Must possess gross motor skills in order to get in and out of vehicle, climb stairs, and fine motor skills to accomplish responsibilities of position and required documentation.
Nursing Supervisor – Extended Hours/Weekend/Rotation
Experienced Home Care nurse who has the ability to influence and direct staff to perform to their maximum potential through communication, education and performance management, who can clearly defines assignments, and foster and maintain an environment of open communication and mutual respect. Requires MA RN License, BSN and previous experience. Scheduled for 2 weekends in a 6 week rotation (Saturday and Sunday 7:30 a.m. to 5:30 p.m.) Includes holiday rotation. On-call triage beeper coverage from 4:30 p.m. the Friday before your assigned weekend through 8 a.m. the Monday following the assigned weekend. Holiday on call triage beeper coverage from 4:30 p.m. prior to the assigned holiday to 8 a.m. the day following the assigned holiday. Notifies On-Call Nurse if a Home visit is required.
NURSING DEPARTMENTS:
Community Health Nurse – Registered Nurse – Full Time# – Caring For Kids (Includes On-Call Rotation)
Requires recent experience in Pediatric and/or Oncology; proficient technical skills with vascular access devices, central and P.I.C.C. lines. Chemotherapy and/or Oncology Certification a plus.
Community Health Nurse – Registered Nurse – Part Time – Extended Hours### (Weekends)
Responsibilities for all community health nurse positions include but are not limited to the promotion of health maintenance of individuals, families and the community through assessment, teaching, counseling and appropriate preventive and rehabilitative nursing measures, working with Agency policies and under the ANA Code of Ethics. Performs similar or related duties as assigned/required. Graduate of NLN nursing program approved by the Massachusetts Board of Registration in Nursing (BSN preferred), current Massachusetts RN license plus one year’s experience in community health nursing and/or one year’s recent acute care medical/surgical nursing experience. Computer literate for documentation into EMR. IV certification and bilingual skills a plus. Must have current driver’s license and a reliable automobile. Must possess gross motor skills in order to get in/out of vehicle, climb stairs, carry community health nursing bag, etc. Fine Motor skills to accomplish skilled nursing care and required documentation.
# Includes working every third weekend plus 1 major winter, 1 major summer holiday and 1 minor holiday
### Work a minimum of four (4) weekend shifts in a six (6) week rotation plus 1 major winter, 1 major summer holiday and 1 minor holiday
MEDICAL SOCIAL WORKER - Part Time (3 days/wk)
Provides for the delivery of social services to adult clients and families. Intervenes and consults with physician, client, family, nursing and rehabilitation staff. Develops the Plan of Care based on social work problems/issues gathered from assessment. Functions as a resource to all teams/departments. Provides referrals to appropriate community services and funding sources. Computer literate for documentation into EMR. Master’s degree in Social Work and current LICSW. Requires at least one year of experience, preferably in community health.
Certified Home Health Aide – Part Time
Works with a team of nurses and therapists to provide personal care to our clients in their homes or place of residence. Performs similar or related duties as assigned/required. Requires satisfactory completion of an approved Home Health Aide training program, certification, and satisfactory completion of a competency test administered by the Brockton VNA. Must have current Mass. Driver’s license and a reliable automobile. Must possess gross motor skills in order to get in and out of vehicle, climb stairs, and fine motor skills to accomplish responsibilities of position and required documentation.
