Director of Rehabilitation Services - Full-Time
Responsible for maintaining quality of patient care within the Rehabilitation Department, in accordance with the Agency's objectives and policies, through planning, coordinating, implementing, and evaluating patient care services. Fosters and maintains an environment of open communication and mutual respect. Organizes department and individual work and tasks in an efficient and cost effective manner. Effectively implements Conditions of Participation. Assists in the development and implementation of the Agency's administrative policies; works within budgetary guidelines; optimizes employee performance. Represents Agency on community committees and organizations/professional organizations. Performs other related duties as assigned or requested. Requirements include: Current Massachusetts License as a Physical Therapist, Occupational Therapist, or Speech Language Pathologist; three to five years of experience as a Physical Therapist, Occupational Therapist, or Speech Language Pathologist, two of which are in community health, and three years in a Leadership Role. Must possess gross motor skills to lift and carry files, equipment, climb stairs, etc. and fine motor skills and close hand/eye coordination needed to operate computer equipment and to perform record keeping and other documentation.
Nursing Supervisor - Extended Hours/Weekend/Rotation
Experienced Home Care nurse who has the ability to influence and direct staff to perform to their maximum potential through communication, education and performance management, who can clearly defines assignments, and foster and maintain an environment of open communication and mutual respect. Requires MA RN License, BSN and previous experience. Scheduled for 2 weekends in a 6 week rotation (Saturday and Sunday 7:30 a.m. to 5:30 p.m.) Includes holiday rotation. On-call triage beeper coverage from 4:30 p.m. the Friday before your assigned weekend through 8 a.m. the Monday following the assigned weekend. Holiday on call triage beeper coverage from 4:30 p.m. prior to the assigned holiday to 8 a.m. the day following the assigned holiday. Notifies On-Call Nurse if a Home visit is required.
Q.A./Clinical Coding Specialist - Full Time
Reviews and analyzes health records to identify relevant diagnoses and procedures. Applies appropriate diagnostic and procedural codes to individual patient health information (PHI) for data entry and retrieval, analysis, and claims processing. Insures the accuracy and completeness of clinical coding. Supervises Quality Assurance Assistants. Requires a clinical background, e.g., LPN,RN, RPT, OT, or SLP, plus the successful completion of a coding program offered by AHIMA, AHCC, or equivalent as a Clinical Coding Specialist (CCS) Certification and two to three years experience with OASIS assessments in the homecare setting, or an equivalent combination of education and experience. Must possess gross motor skills to lift and carry files, equipment, climb stairs, etc. Fine motor skills and close hand/eye coordination needed to operate computer equipment and to perform record keeping and other documentation.

Community Health Nurse - LPN - Per Diem
Responsibilities include but are not limited to the promotion of health maintenance of individuals, families and the community through assessment, teaching, counseling and appropriate preventive and rehabilitative nursing measures, working with Agency policies and under the ANA Code of Ethics. Performs similar or related duties as assigned/required. Graduate of NLN nursing program approved by the Massachusetts Board of Registration in Nursing (BSN preferred), current Massachusetts RN license plus one year's experience in community health nursing and/or one year's recent acute care medical/surgical nursing experience, or current LPN license with at least 2 years home care or acute medical/surgical experience. IV certification and bilingual skills a plus. Must have current Massachusetts's driver's license and a reliable automobile. Must possess gross motor skills in order to get in/out of vehicle, climb stairs, carry community health nursing bag, etc. Fine Motor skills to accomplish skilled nursing care and required documentation.

Medical Social Worker - Part Time (3 days/wk)
Provides efficient and effective delivery of social services to adult clients and families. Intervenes and consults with physician, client, family, nursing, and rehabilitation staff. Develops the Plan of Care based on social work problems/issues gathered from assessment. Functions as a resource to all teams/departments. Provides referrals to appropriate community services and funding sources. Master's Degree in Social Work and current Massachusetts LICSW. Requires at least one year of experience, preferably in community health. Must possess gross motor skills in order to get in/out of vehicle, climb stairs, carry charts, etc. and fine motor skills to accomplish required documentation.
Occupational Therapist Part Time (approx. 12 to 20 visits/week)
Plans, organizes and conducts occupational therapy program in the patient's home and in community programs to facilitate development and rehabilitation of mentally, physically, or emotionally disabled persons. Appropriate Degree and current Massachusetts license required; one-year experience preferred. Must have current Massachusetts driver's license and an automobile. Must possess gross motor skills in order to get in and out of vehicle, climb stairs, and fine motor skills to accomplish responsibilities of position and required documentation.
Certified Occupational Therapist Assistant (COTA) - Part Time
Works in a team relationship with Registered Occupational Therapist in carrying out occupational therapy services for patients in their homes or place of residence. Administers active, active assist and passive manual therapeutic exercises. Current Massachusetts Occupational Therapist Assistant License required; one year of experience required. Must have current Massachusetts driver's license and an automobile.
