Home Health Care is quite simply high quality clinical care provided to you where you live, whether that is in your own home, in someone else’s home, in an assisted living facility or other residential setting. It can often be an alternative to care provided in a hospital, nursing home or rehab facility, giving you the peace of mind of personalized care delivered to where you are most comfortable…in your own home.

There are many programs and insurances that offer a home care benefit. These include Medicare, Medicaid, commercial health insurances, long-term care insurance and the VA. It is always best to verify exactly what is covered by your individual insurance plan. You can call us and we will be happy to help you figure this out.

BVNA nurses are experienced in all aspects of caring for patients, including respiratory care, cardiac care, chemotherapy and oncology, intravenous (IV) therapy, wound care, diabetes, palliative care, and respiratory/pulmonary assessment. All in-home nursing services include care management, symptom management, and pain management.

BVNA Occupational Therapists help patients develop the skills they need to successfully complete day-to-day activities on their own by teaching and practicing everyday tasks, such as grooming, dressing and food preparation.

Vestibular Therapy

Medical Social Work

BVNA Social Workers assess the patient’s physical, emotional, environmental, and social needs and then provides the patient with supportive resources in response to identified concerns and issues. BVNA Social Workers advocate for our patients’ needs with the goal of enhancing our patients’ level of independence.

Home Health Aide Services

Wound Care/Ostomy Care

BVNA’s Wound, Ostomy, & Continence (WOCN/WCC) nurses specialize in the care of patients with wounds, ostomies, or continence disorders. These nurses help to guide BVNA’s practice to create personalized treatment plans for our patients.

  • Wound care nurses assess, treat, and create care plans for patients with complex wounds, ostomies, and/or continence conditions.
  • Wound care nurses also help educate and provide care instructions for patients and families while closely collaborating with their physicians. Specializing in this field allows wound care nurses to direct, coordinate and evaluate patient care to ensure that patients are on the road to recovery. 

Diabetes Care and Education

Brockton Visiting Nurse Association has Certified Diabetes Educators (CDEs) on staff offering high quality diabetes education that is an essential component of effective diabetes treatment. Our Staff follow ADA guidelines and teach the skills necessary for a patient with Diabetes to become independent in managing their condition. General topics of education are:

 

  1. Diabetes basics
  2. Healthy food choices and meal planning
  3. Incorporating physical activity into lifestyle
  4. Medication and insulin teaching
  5. Blood glucose monitoring and interpretation
  6. Preventing acute and chronic complications
  7. Stress management and problem solving

 

  • CDEs provide comprehensive care that gives people with diabetes the knowledge and tools they need to successfully manage their diabetes.
  • Our CDEs will get to know you as an individual, and help you create self-management plans that meet your specific needs based on age, work schedule, daily activities, family demands, eating habits, and health other conditions.

Telehealth Care

Telehealth uses various modes of technology to allow patients to report daily vital signs to BVNA and be engaged with their care plans and self-management.  The patient provides this health information directly from their home and BVNA monitors the patient’s status remotely 7 days a week.

Through the 4G enabled tablets, BVNA is able to track our patients’ conditions and will receive alerts when the patient has high-risk vital sign readings or whose health survey answers raise concerns. Clinicians are then able to quickly intervene either by phone or video chat to help prevent a re-admittance to the hospital.

Each tablet is customized to the patient’s unique needs to take and track their daily vital signs, report their medication adherence, log daily activity, and take a daily customized health survey to see how they are doing.  Additionally, the telehealth nurse can customize educational videos to each patient’s condition so even when there is not a BVNA nurse in the home the patient can continue to learn about their health conditions.
